Metropolitan Maltese Rescue

ADOPTION QUESTIONNAIRE

This comprehensive questionnaire helps us in matching the right dog to the right family. While you might think that our adoption procedures are a bit formal, remember that our dogs have already been separated from a home. As much as possible, we want to insure this will not happen again. An improper placement, or one based on inadequate information, can end tragically for the dog and the adopter family. After your application is approved, you will be placed on our waiting list for a rescued dog.

Please Note:  We only place our rescue Maltese in homes in the area of NYC, Long Island, Southern NY State, Southern CT and Northern NJ. If you live outside these areas please do not apply for adoption.

List ages and relationship to applicant:

Please note:  The Maltese breed is not a good choice for families with children under the age of 10 (or for those planning to start a family) and placement in such a home is against MetroMalts policy.  While we appreciate your interest in a rescue Maltese, please choose a less fragile breed for your family.

Thank you for taking the time to complete this questionnaire.   It is an important part of the adoption process. Before you submit this application, you need to be aware of the following:

HOME VISIT AND ATTACHMENTS:

Each prospective adoptive home will be visited by a Metropolitan Maltese Rescue placement volunteer to evaluate its qualifications and to help assess the right match.

ADOPTION AGREEMENT AND FEE:

After your application is approved and before a dog is placed in your home, you will be asked to sign an adoption agreement which will include, among other things, a statement that you will return the dog to Metropolitan Maltese Rescue if at any time or for any reason you are no longer willing or able to keep the dog in your home.

A fee of $250 for dogs over 4 years old or $400 for dogs under 4 years old is required at the time of adoption.

AVAILABILITY OF RESCUED OR RE-HOMED DOGS:

It sometimes takes several months before the appropriate dog is available for adoption. You should use this time to read books about Maltese (a recently-published one is Bobbie Linden’s "The Maltese: The Owner’s Guide to a Healthy, Happy Pet") and rescued ("Second Hand Dogs" by Carol Lee Benjamin is a good one), surf the internet for applicable sites and check out obedience classes in your neighborhood.

Please be patient - this process is very much like adopting a child. If, in the meantime, your circumstances change and you are no longer able to take a dog, please notify us at adopter@malteserescue.com
